Pharmacology
Ramipril is an angiotensin-converting enzyme (ACE) inhibitor, which prevents ramiprilat's breakdown of the molecule angiotensin I into the vasoconstrictor angiotensin II. Ramipril inhibits ACE, which lowers plasma levels of angiotensin II, vasopressor action, and aldosterone synthesis. Thus, the antihypertensive action of ramipril is realized. Additionally, it can aid in the treatment of heart failure and reduce the risk of myocardial infarction, stroke, and cardiovascular death. It is well tolerated and has a long half-life, making it suitable for long-term use.
Dosage & Administration
The dosage of Ramipril must be adjusted according to the patient's tolerance and response.
Dosage in renal impairment:
Ramipril tablets have to be swallowed with sufficient amounts of liquid. The tablets must not be chewed or crushed. Absorption of Ramipril is not significantly affected by food. Ramipril may, therefore, be taken before, during, or after a meal.
Interaction
Concurrent use with diuretics may result in severe hypotension and, in the case of potassium-sparing diuretics, potentially lethal hyperkalemia. Concurrent lithium treatment may result in a higher blood lithium content. Reduced blood pressure can impair one's ability to drive and operate machinery, and this can be exacerbated by alcohol. NSAIDs may reduce Ramipril's antihypertensive efficacy and worsen renal function.
Contraindications
During pregnancy.
It is best to avoid using ACE inhibitors with extracorporeal procedures that put blood in contact with negatively charged surfaces at the same time because doing so can result in life-threatening allergic responses. These extracorporeal therapies include the use of dextran sulfate for the removal of low-density lipoproteins and certain high-performance membranes (such as polyacrylonitrile) for dialysis or hemofiltration.
Side Effect
Ramipril is well tolerated by the majority of people. Side symptoms such as dizziness, headache, tiredness, and asthenia are frequent. Symptomatic hypotension, cough, nausea, vomiting, diarrhea, rash, urticaria, oliguria, anxiety, and forgetfulness are some of the less common adverse effects. Rare cases of angioneurotic edema, allergic responses, and hyperkalemia have been described.
Pregnancy & Lactation
Ramipril should not be used when pregnant. As a result, pregnancy must be ruled out before beginning therapy. In instances when ACE inhibitor therapy is required, pregnancy should be avoided. If the patient wants to become pregnant, ACE inhibitors must be stopped and replaced with a different type of therapy. If the patient falls pregnant while taking Ramipril, the drug must be changed as soon as feasible with an ACE inhibitor-free therapy plan. There is a danger of damage to the fetus if this is not done. Ramipril should not be taken when nursing.
Precautions & Warnings
In individuals with compromised renal function, hyperkalemia, hypotension, or hepatic function, ramipril should be taken with care.

Store at 30° or below and shield from light. Keep out of the reach of youngsters. Beyond the expiration date, do not use it. It can only be obtained with a valid prescription from a doctor.
